FIRST EVER APPOINTMENT, print the following forms, fill out & bring with you to your scheduled appointment:
- Medical history form (complete both pages)
- Patient information form
- SUBSEQUENT VISITS: 1st visit of each semester please update the patient information form
- If you have any other health insurance aside from your basic school policy, please bring card with you.
